Complete enrollment paperwork and submit required documents with registration fee.

💉 Immunization Records

Up-to-date immunization record from your child's pediatrician.

🏥 Health Assessment

Physical examination form completed within the past 12 months.

🦷 Dental Assessment

Dental examination form completed within the past 12 months for Pre-K students.

🚨 Emergency Contacts

Names and phone numbers for at least two emergency contacts.
